Registration

 

Registration for the More at Four program for the 2008-09 school year begins March 3, 2008. Applications will be available at the Union Smart Start Pre-Kindergarten Office.

Program Eligibility:

  • Children 4 years old on or before August 31 and who will be going to public kindergarten the following year are eligible.
  • Income eligibility must be met for participation in this free program.
  • Priority will be given to children who are not enrolled in quality child care.

Documentation Needed For Pre-Kindergarten Registration:

  • Birth certificate
  • Social Security Card
  • Immunization Record
  • 2 proofs of residence (utility bill, cable, tax bill or lease)
  • Proof of household income (W-2, or IRS 1040)

Requisitos para el Program de Pre-Kinder:

  • Acta de nacimiento
  • Tarjeta de Seguro Social
  • Record original de Vacunas con las vacunas actualizada
  • 2 pruebas de residencia (recibos de utilidad, cable, impuesto, o contrato de arrendamiento).
  • Prueba de ingresos familiar (2007 W-2, รณ Declaracion de Impuestos IRS 1040)
